For your first appointment, please arrive at least 10 minutes early to complete new patient paperwork. Please be sure to bring the following:

  • List of prescription and/or over-the-counter medication, including frequency and dosing
  • Your medical and surgical history
  • Any recent x-rays or relevant medical records.
